Job summary

San Francisco Campus for Jewish Living is seeking an Admissions Coordinator & Intake Coordinator to streamline transitions from hospital to skilled nursing care. You will engage with hospitals, referral partners, and interdisciplinary teams to ensure efficient admissions and exceptional customer service.

You will manage census, support throughput, and coordinate documentation across multiple systems while maintaining high professional standards and confidentiality.

Qualifications

• High school diploma or equivalent required; college degree preferred.
• 1–2 years SNF admissions experience.
• Billing and insurance experience preferred.
• Proficiency with Microsoft Office (Outlook, Word, Excel).
• Ability to multitask in a fast-paced environment.
• Strong interpersonal, organizational, and communication skills.
• Ability to maintain confidentiality and manage relationships with referral sources.
• Experience with admissions processes and data entry.

Responsibilities

• Manage census and patient throughput toward census goals.
• Support data entry across EMR, Allscripts, Curaspan, and other systems.
• Provide phone, email, and customer service support for inquiries.
• Conduct tours and respond to inquiries promptly and professionally.
• Communicate with Case Managers on admissions status and processes.
• Review Long-Term Care applications for completeness and suitability.
• Follow up on outstanding documentation to complete admissions.
• Collaborate with departments to ensure smooth patient transitions.
• Enter admissions into EMR and upload documents.
• Verify insurance eligibility and obtain authorizations as needed.
• Maintain relationships with referral sources and healthcare providers.
• Represent JHRC professionally within the community.
