Job summary

Gracemore Nursing & Rehab is seeking a Health Services Coordinator to manage admissions and communicate available services to referral sources, hospital case managers, physician offices and the community. The role emphasizes accuracy, reliability, and compassionate service for residents and families.

You will coordinate referrals, tours, and pre-admission activity, verify insurance options, and maintain records in line with policy and state/federal guidelines.

Qualifications

• Organized and detail-oriented with strong communication.
• Experience in healthcare sales/marketing/insurance preferred.
• Excellent self-discipline and patient-focused.
• Familiar with medical terminology and PC usage.

Responsibilities

• Coordinate resident/patient referral and approval process.
• Conduct facility tours for prospective residents and families.
• Track pre-admission referral activity daily/weekly.
• Coordinate admission by notifying departments of anticipated admission.
• Ensure paperwork is completed on a timely basis.
• Verify insurance benefits and all payment options.
• Effectively manage occupancy levels and census/premium mix.
• Develop and maintain relationships in the community to inform referral sources of bed availability and ongoing education regarding services.
• Complete records and documentation in accordance with company policy and state and federal guidelines.
• Perform other duties as assigned.
