Job summary

Cumberland Heights Foundation, Inc. seeks a Float Case Manager to join Admissions, coordinating continuing care plans and aftercare across residential programs. You will support detox and direct-admit transitions, respond to AMA, and assist the Medical team with discharge planning.

The role requires strong communication, experience in behavioral health, and the ability to collaborate with treatment teams while maintaining precise documentation in EMR systems. Weekend coverage is included.

Qualifications

• Bachelor's degree in social work, psychology, or related field preferred.
• Minimum two years behavioral health experience; addiction treatment strongly preferred.
• Experience with case management or utilization review is strongly preferred.
• Excellent written and oral communication skills.

Responsibilities

• Initiate continuing care planning at admission and coordinate discharge and aftercare.
• Respond to AMA and engage patients to resolve barriers.
• Provide case management support to Medical teams and Women’s Services when needed.
• Maintain timely documentation in electronic health records and CCP status.
• Coordinate aftercare, referrals, transportation, and housing as required.
