Job summary

Extended Care, LLC is seeking a dynamic Admissions Director to coordinate all admissions and transfers to the facility while marketing our services in the surrounding referral communities. The role ensures a seamless transition for new residents and drives census growth through strategic outreach.

Minimum 1 year of experience in nursing home admissions or marketing is required, with strong knowledge of admission processes and payer sources.

Qualifications

• Minimum 1 year of experience in nursing home or long-term care admissions and/or marketing.
• Strong knowledge of admission processes and payer sources (Medicare, Medicaid, Private Insurance, Managed Care).
• Excellent verbal and written communication skills.
• Proficiency with computer systems and databases.
• Ability to work well in a team with a positive approach.
• Reliable and self-motivated with excellent organizational skills.

Responsibilities

• Oversee the complete admission process for new and returning residents.
• Manage the referral process and build relationships with hospitals, providers, and community partners.
• Conduct facility tours for prospective residents and families.
• Communicate new admissions, discharges, and readmissions to the IDT.
• Meet and greet new residents and family members, obtaining required documentation.
• Collaborate with the Business Office on daily census reconciliation.
• Participate in morning and afternoon meetings to stay informed on census and admissions needs.
• Assist in bed management and room moves as needed.
