Job summary

Bella Terrace is seeking an Admissions Director who is devoted and compassionate about their career. Leadership and effective communication are essential to succeed in guiding referrals and coordinating patient placement.

Responsibilities include building referral relationships, coordinating referrals, ensuring census and revenue goals, and collaborating with physicians, nursing and social work staff to seamless transfers.

Qualifications

• Prior sales and marketing experience in the healthcare industry preferred
• Minimum of two (2) year experience in long-term care

Responsibilities

• Establish and maintain positive relationships with referral sources to promote skilled services
• Coordinate the referral process from the identification of the appropriate payer source to the analysis of patient information
• Responsible for achieving proposed company census and revenue objectives
• Collaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members and social work staff members to ensure seamless transfer to the facility
• Communicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely manner
• Analyze patient needs and match with facility services to plan for an appropriate facility placement
• Complete an on-site visit to the referring facility to review the physician orders, assess the patient’s clinical needs and obtain clinical information
• Other duties as assigned by supervisor
