A skilled nursing and rehabilitation facility in the Hamilton, NJ area is seeking an experienced and compassionate Admissions Coordinator to join our team.
The Admissions Coordinator plays an important role in welcoming new residents and coordinating the admission process from referral through arrival. This position works closely with hospitals, physicians, social workers, discharge planners, families, insurance companies, and our internal clinical team to ensure each admission is handled efficiently and professionally.
Full Time: Tuesday-Saturday EITHER 9A-5P or 10A-6P

Coordinate and facilitate the admissions process for new residents
Receive, review, and follow up on referrals from hospitals, physicians, healthcare providers, and community partners
Collect and verify demographic, insurance, and admission information
Coordinate required documentation and paperwork prior to admission
Communicate with hospitals, discharge planners, case managers, social workers, residents, and families
Help ensure appropriate information is communicated to the nursing and interdisciplinary teams prior to admission
Assist residents and families with questions throughout the admission process
Coordinate admission logistics and help ensure a smooth transition into the facility
Maintain accurate and timely admission records
Assist with insurance verification, authorizations, and related admission requirements
Follow up with referral sources and maintain strong professional relationships
Provide excellent customer service to prospective residents and their families
Work closely with facility leadership and clinical staff to support census and occupancy goals
Maintain confidentiality and comply with all applicable healthcare regulations and facility policies
Previous experience in skilled nursing, long-term care, rehabilitation, hospital, or another healthcare setting
Previous admissions, intake, case management, social services, or healthcare coordination experience preferred
Strong communication and interpersonal skills
Excellent organization and attention to detail
Ability to manage multiple priorities in a fast-paced environment
Professional, friendly, and compassionate approach when working with residents and families
Comfortable communicating with healthcare professionals and referral sources
Knowledge of Medicare, Medicaid, managed care, insurance verification, or post-acute admissions is a plus
Strong computer and documentation skills
Ability to work independently while also collaborating effectively with an interdisciplinary team
