You will collaborate with the Admissions Team and Patient Transition Liaisons, maintain confidentiality, manage records, participate in audits, and contribute to policy development while promoting a positive, inclusive workplace culture.

Qualifications

• High school diploma or equivalent.
• Proficient computer skills and good time management.
• Experience with health insurance, Medicare, Medical Assistance.
• Healthcare experience in SNF or assisted living community.

Responsibilities

• Process incoming SNF referrals and verify insurance eligibility.
• Obtain prior authorizations and document referrals in EMR.
• Maintain departmental records and support audits.
• Promote positive customer service and teamwork.
