Job summary

The Admissions Coordinator at Rehabilitation and Nursing Center in St. Lawrence County will oversee admissions processes, ensure regulatory compliance, and coordinate patient flow from referral to enrollment.

The role is in-person, full-time, with competitive benefits including health and dental insurance, PTO, and a 401(k) plan.

Qualifications

  • 2 years of experience in an Admissions/Marketing position in healthcare environment
  • Long-term care experience required
  • Knowledge of Medicare and 3rd party billing
  • Knowledge of medical terminology

Responsibilities

  • Reconcile Center Census daily
  • Maintain electronic bed board/room assignments and collaborate daily with Environmental team to ensure patient room readiness
  • Communicate all acute readmissions to the team for follow-up, reach out to patient/family to ensure re-admit to the Center
  • Maintain and manage all department data logs and reports- tours, referrals, trend reports
  • Assign and track facility Physician
  • Complete Admissions Checklist and Insurance Verification (Primary and Co-Insurance) processes
  • Check for outstanding balance on all facilities readmissions, prior to accepting all referrals. Collect open balances prior to readmission into the facility.
  • Obtain necessary approvals on financial and clinical criteria for all referrals
  • Input all patient referral information into the necessary systems
  • Communicate all questions and decisions to referral sources/to the team utilizing the appropriate systems
  • Collaborate with the team to gather/collect necessary medical and financial documentation to present to internals teams for approval
  • Complete the required admissions and billing tasks for each patient admission
  • Ensure that patients and family are appropriately welcomed and oriented to the facility on admission day
  • Execute, collect and review all necessary documentation for admissions and Medicaid applications
  • Implement a system to follow up with all inquiries that have not yet been converted to admission. Maintain statistics indicating customer flow, pending and lost inquiries, denials, hospital discharges and marketing activity and submit to regional coordinator as requested
  • Complete admissions agreements for new/readmit patients within 24 hours of admission following Admission
  • Collect appropriate insurance cards
  • Works with the Administrator and facility staff to ensure consistent, effective admissions, discharge and transfer meetings are conducted for all residents

OVERVIEW

The admissions Coordinator reports to the Administrator. The admissions Coordinator will ensure that regulatory compliance is met by ensuring that activities are completed in accordance with the standard operating procedures as outlined by the organization.

RESPONSIBILITIES
