Job summary

Goldwater Care in Illinois is seeking an Admissions Liaison to be the welcoming first point of contact for prospective residents, families, and referral sources throughout inquiry, assessment, intake, and admission.

You will conduct bedside assessments, verify demographic and financial information, and coordinate the admission process with residents and facility staff, while maintaining confidentiality and delivering professional service.

Qualifications

• Must have at least 2 years of experience in a healthcare setting.
• Effective oral and written communication.
• Strong organizational and time management skills.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Serve as a primary point of contact for prospective residents, families, representatives, referral sources, and community partners, providing a welcoming and professional experience throughout the inquiry, assessment, intake, and admission process.
• Conduct bedside assessments and referrals at hospitals, healthcare facilities, and other locations in collaboration with prospective residents, representatives, families, case managers, social workers, and other healthcare professionals to evaluate appropriateness for admission.
• Gather and verify necessary demographic, financial, medical, and referral information to support accurate, timely, and compliant admissions.
• Coordinate and facilitate the admission process, including communicating with prospective residents, families, referral sources, and facility staff regarding admission requirements, placement, and available services.
