Premier Healthcare LLC is seeking an Admissions Coordinator/Liaison for our Ohio nursing home. The role focuses on coordinating the intake process, collecting and verifying documentation, and communicating with patients, families, and healthcare providers to provide timely admission updates.

You will transfer patient records, build relationships with healthcare partners, and ensure a positive admissions experience while complying with regulatory requirements and internal policies.

Qualifications

• Prior experience as Admissions Coordinator in a nursing home.

Responsabilités

• Coordinate the intake process, ensuring all necessary documentation and information are collected and accurately processed.
• Communicate with patients, families, and healthcare providers to gather information and provide updates on admission status.
• Facilitate the transfer of patient records and information to our facility.
• Develop and maintain relationships with healthcare providers and community partners to ensure timely and efficient admission processes.
• Provide exceptional customer service, addressing patient and family concerns, and ensuring a positive experience during the admissions process.
• Collaborate with the Admissions team to ensure compliance with regulatory requirements and organizational policies.
• Participate in process improvements and quality initiatives to enhance the admissions experience.
