Job summary

Spring Creek Care Center in Millcreek, Utah, is seeking an Admissions Coordinator to join a dedicated team. This full-time role involves front-line customer service, facility tours, and coordinating resident admissions with internal and external partners.

You will support residents in care planning, complete admission paperwork, and help orient new residents while maintaining accurate EMR data and cross-department communication. Excellent communication and empathy are essential.

Qualifications

• 1 year of clerical experience in a skilled nursing facility preferred.
• Strong communication, customer service skills, and empathy.
• Proficient with computers, office systems, and related software.
• Working knowledge of health insurance processes and systems.
• Self-motivated with a willingness to assist across tasks.

Responsibilities

• Provide front-line customer service, including courteous phone coverage and daily quality assurance rounds.
• Conduct facility tours for prospective residents, families, and responsible parties.
• Coordinate resident admissions with internal departments and external healthcare partners.
• Build and maintain referral relationships with discharge planners, case managers, and physicians.
• Complete admission paperwork and support residents in care planning and decision-making preferences.
• Assist with resident orientation in accordance with facility procedures.
• Enter and maintain accurate information in EMR and other systems.
• Support communication between departments to ensure smooth admissions and transitions.
• Complete additional duties as assigned.
