• Maintain communication with hospital social workers and discharge planners.
• Prepare admissions packets and obtain necessary signatures and forms.
• Manage census data and monthly reports for administration.
• Assist with marketing efforts to promote facility admissions.
• Attend meetings and collaborate with hospital and community partners.
• Serve as weekend supervisor as part of department rotation.
