Key duties include coordinating admissions, verifying insurance, and maintaining strong relationships with referral partners while providing excellent customer service to families and residents.

Qualifications

• Minimum 2 years of admissions experience in a Skilled Nursing Facility.
• Strong understanding of Medicare, Medicaid, PDPM, and payer guidelines.
• Experience using Electronic Medical Record (EMR) systems.
• Excellent communication and organizational skills; able to build relationships.
• Ability to multitask in a fast-paced healthcare environment.

Responsibilities

Rehabilitation Center of Winter Park is seeking a motivated, detail-oriented, and compassionate Admissions Coordinator to join our growing team. In this key role, you will serve as the primary liaison between hospitals, referral sources, residents, and families, ensuring a seamless admissions process while helping drive occupancy growth and exceptional customer service.

Key Responsibilities

• Coordinate and facilitate the admissions process from referral through admission.
• Collaborate with hospitals, physicians, case managers, and referral sources to ensure smooth resident transitions.
• Develop and maintain strong relationships with referral partners and community organizations.
• Support marketing and outreach efforts by attending networking events and community functions.
• Educate prospective residents and families regarding services, policies, visitor guidelines, and admission procedures.
• Verify insurance coverage, financial information, and authorization requirements prior to admission.
• Maintain working knowledge of Medicare, Medicaid, Managed Care, and payer-specific guidelines.
• Monitor admissions activity and prepare monthly reports on admissions trends and census growth.
• Identify operational barriers affecting admissions and work with leadership to implement solutions.
• Communicate admission details effectively with interdisciplinary team members to ensure continuity of care.

Qualifications

• Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF) required.
• Strong understanding of Medicare, Medicaid, Managed Care, PDPM, HMO guidelines, and RUGS II & III.
• Experience working with Electronic Medical Record (EMR) systems.
• Knowledge of CMS regulations and Department of Health (DOH) requirements.
• Excellent communication, organizational, and relationship-building skills.
• Ability to multitask and work effectively in a fast-paced healthcare environment.
