Ambassador Healthcare at College Park is seeking a motivated, detail-oriented, and compassionate Admissions Coordinator to join our growing team. In this key role, you will serve as the primary liaison between hospitals, referral sources, residents, and families, ensuring a seamless admissions process while helping drive occupancy growth and exceptional customer service.
Coordinate and facilitate the admissions process from referral through admission.
Collaborate with hospitals, physicians, case managers, and referral sources to ensure smooth resident transitions.
Develop and maintain strong relationships with referral partners and community organizations.
Support marketing and outreach efforts by attending networking events and community functions.
Educate prospective residents and families regarding services, policies, visitor guidelines, and admission procedures.
Verify insurance coverage, financial information, and authorization requirements prior to admission.
Maintain working knowledge of Medicare, Medicaid, Managed Care, and payer-specific guidelines.
Monitor admissions activity and prepare monthly reports on admissions trends and census growth.
Identify operational barriers affecting admissions and work with leadership to implement solutions.
Communicate admission details effectively with interdisciplinary team members to ensure continuity of care.
Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF) required.
Strong understanding of Medicare, Medicaid, Managed Care, PDPM, HMO guidelines, and RUGS II & III.
Experience working with Electronic Medical Record (EMR) systems.
Knowledge of CMS regulations and Department of Health (DOH) requirements.
Excellent communication, organizational, and relationship-building skills.
Ability to multitask and work effectively in a fast-paced healthcare environment.
