Job summary

The Hazelden Betty Ford Foundation is seeking a Patient Placement Counselor to assess applicants for appropriate levels of care and coordinate pre-admission needs. You will work with applicants, referents, families and the clinical team to ensure smooth transitions from inquiry to admission.

Responsibilities include conducting assessments, documenting in the EHR, crisis management, and coordinating with internal and external resources while adhering to confidentiality and HIPAA policies.

Qualifications

• Bachelor’s degree required; Master’s degree preferred for higher level.
• Certificate/degree in addiction counseling or equivalent.
• Must be 21 or older; CPR certification or within 30 days.

Responsibilities

• Meet with applicants to assess diagnosis and level of care.
• Provide pre-admission education and referrals within the Hazelden Betty Ford system.
• Document accurately in the EHR and communicate with the clinical team.
• Offer crisis management services as needed.
• Coordinate information sharing with internal and external resources.
