Job summary

The Hazelden Betty Ford Foundation is seeking a Patient Placement Counselor to perform assessments with applicants to determine the appropriate program and level of care. Qualifications

• Bachelor’s degree or higher in a relevant field.
• Master’s degree in behavioral health preferred.
• Certificate/degree in addiction counseling or equivalent required.
• CPR certification or ability to obtain within 30 days (AHA or Red Cross).
• LADC license in MN required.

Responsibilities

• Meet with qualified applicants to complete assessments to determine program appropriateness and level of care.
• Create a client-focused experience for all clients and referents seeking services.
• Document accurately and timely in the EHR.
• Communicate clearly with clinical teams and others.
• Provide crisis management to prevent or reduce emergencies.
• Share patient information with internal/external resources per policy and HIPAA.
