Job summary

The Avante Group in New York is seeking an Internal Admissions Director for our Skilled Nursing Facility. You will lead patient placements for Short Term Rehabilitation, Respite, or Long-Term Care, coordinating with Care Navigators and Nurse Liaisons at hospitals.

The role requires excellent communication, knowledge of Medicare/Medicaid, and up-to-date SNF admissions regulations. You will partner with physicians and referral partners, explain benefits, and contribute to a team-based marketing

Qualifications

• Bachelor’s Degree or equivalent in related field.
• 3–5 years experience in Long Term Care or similar healthcare setting.
• Knowledge of Medicare, Medicaid and other insurances.
• Familiar with Federal and State SNF admissions regulations.
• Ability to communicate effectively, orally and in writing.
• Ability to work flexible hours and weekends.
• Proficient with Outlook, Excel, Word.
• Must be at least 18 years of age.
• Knowledge of healthcare terminology.

Responsibilities

In this role, as Internal Admissions Director, you will be the frontline to our patients and families looking for placement within our Skilled Nursing Facility for Short Term Rehabilitation, Respite or Long-Term Care. You will connect with our Care Navigators and Nurse Liaisons within the hospitals. You will communicate via electronic referral system(s) and calls with our referral partners that will include Physicians, Hospital Case Managers, Hospice Organizations, Home Health Organizations and Health Plan Referral Departments. Your role is vital as you will ensure that benefits are acceptable and explained to potential admissions. You will represent the facility with the area hospitals and other medical professionals. You will be a member of the marketing team and an important part of the overall team-approach in the facility.

• Must possess the ability to communicate effectively, orally and in writing.
• Must possess the ability to deal tactfully with personnel, residents, visitors and the general public.
• Must have knowledge of medical terminology.
• Must have knowledge of Medicare, Medicaid and other insurances.
• Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing admission practices.

• Must understand and be up to date with the Federal and State regulations for SNF’s related to admissions.
• Ability to work flexible hours and weekends
• Ability to read and write in English
• Must be at least 18 years of age.
• Must have a Bachelor’s Degree or equivalent experience in related field
• 3- 5 years’ experience in Long Term Care, which can include an SNF, ALF, Hospital or similar healthcare organization and in a similar titled position
• Knowledge of healthcare Federal and State regulatory standards.
