Job summary

Lakeport Post Acute is seeking an Admissions Assistant to support our admissions team and ensure a seamless transition for residents and families. You will coordinate admissions, verify insurance details, and maintain accurate documentation in a confidential environment.

This in-person, full-time role requires strong communication, organization, and interpersonal skills to interact with residents, families, hospitals, and referral sources.

Qualifications

• High school diploma or equivalent.
• Strong communication and customer service skills.
• Excellent organizational skills and attention to detail.
• Proficiency with Microsoft Office applications, including Word, Excel, Outlook, and Teams.
• Ability to maintain professionalism and confidentiality.

Responsibilities

• Serve as the first point of contact for prospective residents, families, hospitals, and referral sources.
• Coordinate and schedule admissions, including pre-admission assessments and required documentation.
• Maintain accurate resident records in compliance with federal, state, and facility regulations.
• Assist with insurance verification, prior authorizations, and financial eligibility processes.
• Conduct facility tours for prospective residents and families.
• Prepare and distribute admission packets and related paperwork.
• Collaborate with nursing, social services, therapy, and administrative teams to ensure a smooth admission process.
• Follow up with hospitals, physicians, case managers, and referral sources to obtain and exchange required information.
• Support census development, marketing initiatives, and community outreach efforts as needed.
• Maintain strict confidentiality and compliance with HIPAA regulations.
• Perform other duties as assigned.
