Qualifications

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC or sub-acute rehab preferred.
• Experience handling admissions, discharge coordination, or desk nursing is desirable.
• Hands-on EMR experience (PointClickCare) for rapid chart initiation.
• Excellent organizational and interpersonal communication with families and hospital staff.

Responsibilities

• Lead clinical intake and onboarding during the evening shift (3:00 PM – 11:00 PM).
• Conduct initial nursing assessments and admission documentation.
• Reconcile hospital discharge summaries and orders for continuity of care.
• Coordinate with discharge planners, care teams, and families for room setup and equipment.
• Initiate preliminary care plans and relay needs to on-duty supervisors.
• Orient new residents and families to facility policies and routines.
