Responsibilities

• Provide individual and group counseling to patients assigned to caseload.
• Direct training groups and seminars; lead therapeutic sessions.
• Coordinate referrals with community agencies and law enforcement for continuity of care.
• Complete intake assessments and develop individualized treatment plans with patient input.
• Maintain patient charts in DHHS-compliant formats; protect confidentiality.
• Assist with urine sample collection and documentation for drug screens.
• Participate in CQI activities and treatment planning discussions.
