Job summary

HeartShare Wellness LTD in New York City seeks a Floating Case Planner to assist case planners with day-to-day social services tasks and support services for children in foster care. You will connect families to resources, help reunification efforts, and cover for colleagues as needed.

The role includes developing service plans with families, coordinating visits, attending court hearings, and collaborating with medical and mental health teams to ensure safety and permanency.

Qualifications

• Bachelor's degree required.
• Knowledge of Child Welfare is a plus.
• Strong communication and collaboration skills.

Responsibilities

• Create service plans with child and families addressing safety, behavioral, educational, psychological and permanency needs.
• Monitor safety, well-being and progress via regular case contacts and documentation.
• Schedule and supervise mandated visits; conduct interviews with child and families; perform field visits to homes, schools, courts, prisons, hospitals, and other sites.
• Attend weekly supervision and follow supervisor directions for planning and follow-up.
• Attend Family Court hearings and provide testimony; follow court directives.
• Participate in Family Team Conferences to advance permanency for children and families.
• Coordinate with medical, mental health and case management to monitor needs and ensure services are provided.
• Utilize program data to measure performance and ensure outcomes are met.
• Serve as alternate support for Family Team Conference coverage.
• Perform other duties as assigned and compliant with laws and policies.
