Soleo Health is seeking a Therapy Care Pharmacist to support our Acute Therapy Care Management Team in Tempe, AZ. The role focuses on collecting clinical data, implementing and monitoring care plans, and collaborating with prescribers to optimize therapy for specialty medications.

You will supervise therapy care specialists, ensure compliance with state and federal requirements, and participate in training, on-call rotations, and QA initiatives.

Formación

• Experience in home infusion or specialty pharmacy preferred.
• Doctor of Pharmacy (Pharm D) or R.Ph with clinical experience.
• Active state licensure and relevant residency/training encouraged.

Responsabilidades

• Collect clinical data for patients, assess efficacy and safety of therapies.
• Collaborate with prescribers to implement or modify treatment plans.
• Serve as a clinical knowledge resource to patients and staff.
• Coordinate with pharmacy team to ensure efficient, patient‑centered care.
• Document all patient interactions in the designated system.
• Dispense specialty medications and supplies.
• Support QA and Performance Improvement initiatives.
• Assist in orientation and training of new staff.
• Supervise therapy care specialists and support personnel.
• Ensure compliance with state/federal law and accreditation standards.
• Obtain additional state pharmacy licenses as requested.
