Job summary

Soleo Health in Hoover, AL is seeking a Full-time Therapy Care Pharmacist to support our Acute Therapy Care Management Team. You will collect clinical data, implement care plans, and collaborate with prescribers to optimize patient outcomes while dispensing specialty medications.

This on-site role requires a Pharm D or R.Ph, active state licensure, and experience in home infusion or specialty pharmacy. Schedule is 40 hours weekly, Monday to Friday, with optional on-call.

Qualifications

• Pharmacist with strong clinical and patient-service focus.
• State licensure must be active.
• Residency/training in clinical pharmacy is preferred.

Responsibilities

• Collect patient data and assess therapy efficacy and safety.
• Collaborate with prescribers to modify treatment plans.
• Act as clinical resource for patients and staff.
• Coordinate with pharmacy team to ensure efficient care.
• Document interactions and care coordination clearly.
