Soleo Health is seeking a Full-time Therapy Care Specialist to support our Acute Infusion Pharmacy patients in Tempe, AZ. The Therapy Care Specialist will work under the supervision of a Pharmacist to oversee logistics of a patient’s dispense and prescription maintenance, data capture, and coordination across the patient care continuum while on service with Soleo Health.

Qualifications

• High School Diploma or equivalent required.
• Arizona Pharmacy Technician license required.
• PTCB or NHA certification required.

Responsibilities

• Act as single point of contact for specialty patients and coordinate medication dispense and delivery with branch staff.
• Capture assessment data, ascertain patient supply needs, and schedule new/refill deliveries.
• Run pharmacy reports daily/weekly as needed.
• Answer phones, transfer calls, and monitor faxes and refill queues.
• Coordinate insurance coverage and authorizations with intake/billing staff.
• Document all patient interactions and care coordination in the system.
• Complete therapy-specific training as required.
