Job summary

Soleo Health is seeking a Full-time Therapy Care Pharmacist to support our Acute Therapy Care Management Team in Hoover, AL. The Pharmacist will collect and assess clinical data, implement and monitor care plans, and dispense specialty medications while coordinating with prescribers to optimize patient outcomes.

The role requires a Pharm D or R.Ph with clinical experience, active licensure, and a willingness to work on-site with occasional on-call duties.

Qualifications

• Pharm D or R.Ph with relevant clinical experience.
• Post graduate residency and/or training.
• Active state licensure.
• Experience with CPR+ pharmacy system preferred.
• Excellent clinical and customer service skills.
• Proficiency with Excel, Word and Outlook.

Responsibilities

• Collect clinical data for patients and assess therapy efficacy and safety.
• Collaborate with prescribers to modify treatment plans.
• Act as a clinical knowledge resource for the team.
• Coordinate activities with pharmacy and care staff for patient outcomes.
• Document interactions in the designated system of record.
• Dispense specialty medications and supplies.
• Supervise therapy care specialists and support staff.
• Ensure compliance with laws, regulations and payor requirements.
