Job summary

Soleo Health, Inc. is seeking a Therapy Care Pharmacist in Tempe, AZ to support the Acute Therapy Care Management Team. The role involves clinical data review, care plan implementation, and collaboration with prescribers to optimize patient outcomes.

Eligible candidates hold a PharmD or R.Ph with licensure and relevant residency or training, with preference for home infusion or specialty pharmacy experience. The position is full-time, on-site with standard weekday hours.

Qualifications

• PharmD or R.Ph with active state license.
• Postgraduate residency or equivalent clinical training preferred.
• Experience in home infusion or specialty pharmacy is advantageous.

Responsibilities

• Collect and assess clinical data to monitor efficacy and safety of therapies.
• Collaborate with prescribers to implement or modify treatment plans.
• Provide clinical expertise to patients, prescribers, and team members.
• Coordinate activities with pharmacy and care teams to ensure optimal outcomes.
• Document interactions and care coordination in the system of record.
• Dispense prescribed medications and supplies and supervise staff.
