Job summary

Soleo Health is seeking a Full-time Therapy Care Pharmacist to support our Acute Therapy Care Management Team in Hoover, AL. This role requires a Pharm D or R.Ph with clinical experience and active licensure, plus parenteral certification.

You will work on-site, coordinating with a national team to deliver high-quality specialty pharmacy care. Responsibilities include data collection, collaboration with prescribers, and supervision of therapy care staff to ensure optimal patient outcomes and

Qualifications

• Prior experience in home infusion and/or specialty pharmacy preferred.
• Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience.
• Post graduate residency and/or training.
• Applicable active state licensure.
• Parenteral Certification Required.
• Additional specialty certification or training may be required.
• Prior experience with CPR+ pharmacy computer system preferred.
• Excellent clinical and customer service skills.
• Basic to intermediate efficiency with Microsoft Excel, Word and Outlook.

Responsibilities

• Collect and assess clinical data from patients, caregivers, and nurses to monitor efficacy and safety of prescribed therapies.
• Collaborate with prescribers to implement or modify treatment plans.
• Act as a clinical resource to patients, prescribers, and team members.
• Coordinate activities with pharmacy team and other staff to ensure efficient, patient-centered care.
• Document interactions and care coordination in the system of record for clear communication.
• Dispense specialty medications and supplies.
• Support quality assurance and performance improvement initiatives.
