Job summary

Brio Living Services seeks an Acute Home Care PCA in St. Joseph, MI to deliver flexible, high-quality in-home and day center support. You will assist participants with daily living activities, support clinical teams, and provide hands-on training to home health staff while on the road.

The role emphasizes continuity of care and safety, with Monday–Friday 8am–4:30pm, 40 hours per week, reporting to Teresa Ford. A valid Michigan driver’s license and CPR/BLS are required or obtainable.

Qualifications

• CNA certification is required.
• One year with frail or elderly individuals preferred.
• Experience in home health, long-term care, acute care, or similar settings.

Responsibilities

• Provide direct care to participants in their homes, assisting with ADLs and basic health monitoring.
• Follow care plans and communicate observations to nursing/clinical staff.
• Support independence and well-being in the home environment.
• Assist day center participants with ADLs, meals, mobility, social engagement, and supervision.
• Provide support to day center clinical team during peak times.
• Maintain a safe, clean, and supportive environment.
• Document care provided and participant status changes.
• Report health or safety concerns to the care team promptly.

Schedule: 40 hours per week | 8am - 4:30pm | Monday - Friday

Department: PACE of Southwest Michigan | St. Joseph, MI

What You'll Do: The Acute Care PCA provides flexible, high-quality support across multiple care settings, including in-home care, the day center, and training home care team members. This role ensures continuity of care by assisting participants with daily living activities, supporting clinical team members, and delivering hands-on training to home health team members while on the road.

Key Responsibilities

• Provide direct care to participants in their homes, including assistance with activities of daily living (ADLs), basic health monitoring, mobility support, and safety checks.
• Follow individualized care plans and communicate observations to nursing or clinical staff.
• Support participants in maintaining independence and well-being within their home environment.
• Assist participants attending the day center with ADLs, meal assistance, mobility support, social engagement, and general supervision.
• Provide additional support to day center clinical team members during peak times.
• Maintain a safe, clean, and supportive environment for all participants.
• Assist in delivering hands-on training, coaching, and support to home health team members in the field.
• Demonstrate proper care techniques, safety protocols, and participant support strategies.
• Document care provided, observations, and participant status changes.
• Report concerns related to participant health, safety, or functional status to the care team promptly.

Skills, Knowledge and Expertise:

• CNA certification, required.
• One (1) year of prior experience with frail or elderly individuals, aiding with daily living activities.
• Experience providing direct care in home health, long-term care, acute care, or similar settings.
• Valid Michigan Driver’s License with a clean driving record and reliable transportation.
• Current CPR or BLS certification or willing to obtain.
