Job summary

Qualifications

• Mandatory Reporter training within 3 months of hire.

Responsibilities

• Conducts initial assessment within 30 minutes of arrival.
• Develops outcome-based care plans.
• Collaborates with physicians, families and care teams.
• Documents discharge instructions and plan.
• Maintains patient safety and medication administration standards.
