Qualifications

• Current Iowa RN license required.
• BSN preferred.

Responsibilities

• Conducts an initial assessment to gather data and admissions.
• Develops outcome/goal-based plans of care and prioritizes patient needs.
• Implements plans of care, coordinates with the care team and physicians.
• Coordinates discharge planning and ensures safe transitions.
• Maintains clear documentation and communicates with the interdisciplinary team.
