Job summary

Novant Health's Rowan Medical Center in Salisbury, NC is seeking an experienced RN to join a patient-facing, non-bedside discharge planning team. This role offers exposure to diverse patient populations and a collaborative environment.

You will lead complex discharge planning, work with physicians and MSW case managers, coordinate care, and ensure safe post-acute options, while maintaining community resources knowledge.

Qualifications

• 2 Year / Associate Degree, required.
• Associates Degree in Nursing or BSN, required.
• 4 Year / Bachelor’s Degree, preferred.

Responsibilities

• Complex discharge planning.
• Collaborate with physicians and healthcare team to assess, plan, implement, and evaluate discharge plans.
• Collaborate with MSW case managers for psychosocially complex cases.
• Facilitate coordination of care.
• Accept responsibility, authority, and accountability for management of care.
• Serve as a resource for discharge planning and care coordination for medically complex cases.
• Maintain knowledge of community resources and post-acute care options.

What You’ll Do

• Complex discharge planning
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
• Collaborate with MSW case managers for guidance with pscyhosocially complex cases
• Facilitate coordination of care
• Accept responsibility, authority, and accountability for management of care
• Serve as a resource for discharge planning and care coordination for medically complex cases
• Maintain current knowledge of available community resources/post acute care options

What We’re Looking For

• Education: 2 Year / Associate Degree, required. Associates Degree in Nursing or BSN, required. 4 Year / Bachelors Degree, preferred.
• Experience: Case Management experience, preferred. Broad background of bedside care strongly preferred.
• Licensure/Certification: Current RN licensure in state of employment, required.
• Additional skills required: Weekend and holiday coverage.
