Qualifications

• Licensure as Physical Therapist in the state of practice.
• BLS certification preferred or required within set timeframe.

Responsibilities

• Evaluates patients and develops individualized treatment plans.
• Provides direct therapy and instructions to patients and families.
• Maintains accurate progress notes and coordinates care in the EMR.
• Maintains productivity while ensuring quality of care.
• Supervises/PT aides and ensures compliance with safety protocols.
