Job summary

Advocate Sherman Hospital - Acute Care Rehabilitation is seeking a Part-time Physical Therapist to evaluate and treat patients in an acute care setting. The role requires weekend shifts, the ability to work with a team, and commitment to high-quality care.

Licensure in the state and a Doctorate in Physical Therapy are required; EPIC EMR experience is preferred but not mandatory. Opportunity to cross-train and cover PTO or leaves may arise.

Qualifications

• Doctorate in Physical Therapy required and license to practice in the state.
• Experience in an acute care setting preferred; 1 year preferred.
• Excellent communication and organizational abilities required.

Responsibilities

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Develop and implement plan of care with goals; monitor outcomes and update physicians and team.
• Plan discharge with home programs and family education; coordinate with community services.
• Document all patient care per policy with co-signatures and complete daily statistics and scheduling.
