Job summary

Advocate Health Lincoln - Physical Therapy is seeking a part-time Physical Therapist to provide direct therapy services to in-patients across a seven-day schedule. The role includes evaluating patients, creating comprehensive care plans, and coordinating treatment and discharge planning with the care team.

The candidate must hold a Bachelor's in Physical Therapy, a valid state license, and BLS certification.

Qualifications

• Bachelor's degree in Physical Therapy program required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Provides direct therapy services to patients admitted to a level of care requiring 7 days a week service.
• Evaluates patients and develops patient care plans including modalities and education.
• Gathers data from medical records, physician reports, patient and family as needed and documents.
• Documents evaluations, tests, progress notes, daily treatment notes, education, outcomes and charges.
• Communicates evaluation and progress to physicians, referrals and other staff; coordinates schedule and discharge planning.
• Provides supervision of PTA's and Rehab Tech/Aide per licensure requirements.
