Qualifications

• Licensed Physical Therapist (PT) in good standing.
• BLS within 30 days (AHA).
• Complete required continuing education for acute stroke care.

Responsibilities

• Perform patient transfers with appropriate body mechanics.
• Provide compassionate, respectful patient care and engagement.
• Design and adjust care plans based on goals, outcomes, and response.
• Educate patients, families, and other providers.
• Evaluate findings to determine PT diagnoses and outcomes.
• Conduct examination using appropriate methods.
• Document results per CTSCP67 policy.
• Screen patients to determine therapy needs and effectiveness.
• Supervise and provide feedback to support staff.
• Complete annual education for applicable program requirements.
