TriHealth is actively recruiting an activities coordinator for our new PACE of Cincinnati PACE Center located in Norwood Ohio. Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

Job Overview:

The PACE Activities Coordinator is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing recreational therapy assessments for new PACE participants and reassessments for existing PACE participants as part of the PACE IDT care planning process. The coordinator provides a comprehensive therapeutic recreation and activity program, primarily at the PACE Center, with possible community outings.

Job Requirements:

• Bachelor's Degree in Recreational Therapy from an accredited college (Preferred)
• High School Diploma or GED (Required)
• 2-3 years experience in social or recreational programs serving the frail or elderly, including at least one year full-time in a healthcare setting (Required)
• 1-2 years experience working with a frail or elderly population; training provided if lacking experience (Required)
• Experience with Electronic Records
• Reliable Transportation
• BLS certification (Required)

Job Responsibilities:

• Participate as a PACE IDT member, conducting initial and reassessment recreational therapy evaluations
• Develop and implement realistic, measurable recreation goals in collaboration with the IDT, considering medical, physical, social, and emotional needs
• Supervise recreation aides and staff as directed
• Attend IDT and team meetings, reporting on participant engagement and status
• Monitor health or psychosocial changes requiring reassessment, communicating changes promptly
• Document in the PACE EHR per regulations and policies
• Support and participate in PACE QI activities
