Department Description

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). It serves adults 55 years and older with chronic medical and/or mental health difficulties, aiming to maximize community independence and prevent nursing home placement. Our interdisciplinary team works to integrate medical, behavioral health care, community resources, and in-home support services.

Job Purpose

The Activities Coordinator I creates and delivers engaging, therapeutic, and person-centered activities for PACE participants. The role involves facilitating group and individual activities that promote physical, cognitive, emotional, and social well-being, collaborating closely with the Activities Director and participants to ensure activities are meaningful and inclusive.

Key Responsibilities

  • Design, plan, and implement daily activity programs tailored to older adults' diverse needs
  • Lead therapeutic activities across physical, cognitive, spiritual, and emotional domains
  • Orient new participants to the activity program
  • Assist in organizing field trips for participants
  • Collaborate with Personal Care Assistants on day center operations, including beverage service, meal assistance, and transportation
  • Manage behavioral issues and conflicts professionally
  • Maintain monthly activity calendars and supply orders

Qualifications

  • High school diploma or equivalent; BA or BS preferred
  • Fluency in Spanish, Portuguese, and/or Haitian Creole is highly desirable
  • At least one year of experience working with frail elders in a social or medical setting
  • Experience in developing and conducting therapeutic activities
  • Activity Director Certification from the National Council of Activity Professionals required within 6 months of hire
