Brief Summary Of Purpose
Serves as a member of the PACE interdisciplinary team, assessing needs, interests, and capabilities of participants, and developing individualized therapeutic recreational care plans based on diagnoses and assessments. Plans, organizes, directs, and participates in a comprehensive therapeutic recreation program tailored to participants' needs, promoting mental health, social connections, and physical health within an "active aging" mindset.

Responsibilities
Job Responsibilities:

  1. Conducts various therapeutic recreational groups and individual activities to improve or maintain participants' cognitive, physical, social, psychosocial, spiritual, and artistic needs.
  2. Contributes activity ideas for monthly planning calendars.
  3. Demonstrates programming for educational and culturally competent activities such as exercise, arts and crafts, music, and spirituality.
  4. Ensures participant choices and engagement in activities.
  5. Builds a community and social atmosphere, treating participants with respect and dignity, providing support at the ADHC.
  6. Maintains cleanliness of supplies and equipment, reporting damage or safety issues.
  7. Suggests supplies or equipment beneficial for activities.
  8. Participates in interdisciplinary team meetings and case conferences as assigned.
  9. Informs supervisor of changes in participants' conditions or complaints.
  10. Completes work reliably within set timeframes.
  11. Documents attendance and participation daily.
  12. Communicates effectively with participants, staff, and external contacts.
  13. Attends staff and other meetings as required.
  14. Performs all duties in accordance with policies and procedures.
  15. Completes activity assessments as needed within regulatory timelines.

Qualifications

  • High school diploma or equivalent.
  • Reliable transportation for travel within the service area.
  • One or more years of experience in activities programs for elderly or frail populations, or completion of Fallon Health training.
  • CPR and Alzheimer's certifications or willingness to obtain them.
