Description

Participate in planning and conducting individual, small, and large group activities. Assist in providing effective communication between employees at all levels, residents, their families, support personnel, government agencies, and the public to meet the needs and best interests of residents, the community, and the facility. Engage in community planning related to the facility's interests and the needs of residents and their families. Assist in developing the monthly activity calendar and maintaining attendance records. Support the Quality Assurance and Assessment Committee in developing and implementing plans to address quality deficiencies as requested by the Activities Director. Participate in discharge planning, develop and implement activity care plans, and conduct resident assessments. Arrange transportation for residents for outings, appointments, or discharges. Develop a monthly activity schedule that includes outings, scheduled group activities, and in-room activities for bed-bound or isolated residents. Encourage residents to participate in self-initiated activities such as hobbies, crafts, and reading, providing materials as needed, including Braille or audiobooks. Assist with documentation, quarterly progress notes, and maintain a clean, orderly, and secure Activity Department. This position has no supervisory responsibilities.

Qualifications include a high school diploma or equivalent, with preference given to candidates with at least one year of experience in a long-term care facility. The ability to read and comprehend policies, present information effectively, and apply mathematical concepts is required. Physical demands include frequent standing, walking, reaching, pushing/pulling, talking, hearing, tasting, smelling, and lifting up to 50 pounds, with occasional climbing, stooping, and crouching. The work environment is typically low to moderate noise, with accommodations available for individuals with disabilities. The employer reserves the right to reassign duties as needed.
