The position includes supervision of ACT staff, community-based direct care, and collaboration with community partners to monitor and develop the program. Responsibilities include leading team meetings, supervising staff, ensuring fidelity to ACT practices, handling referrals, documenting records, and participating in recruitment, budgeting, and agency committees.

Responsibilities

• Lead ACT team meetings with prescriber and ACT specialist to promote collaboration.
• Provide individual supervision and staff orientation to program philosophy and goals.
• Monitor fidelity to ACT measures and best practices.
• Oversee referrals and staff assignments.
• Assess patient health needs and provide outreach when needed.
• Ensure timely documentation per agency standards.
• Provide crisis intervention and direct care in the community as needed.
• Assist in recruiting and hiring processes with program director.
• Manage program budget and monitor costs and data.
• Represent Lakes Region Mental Health Center in community and on committees.
