Family Health Services in Idaho is seeking a dedicated team member to join the ACT multidisciplinary group, helping patients achieve recovery goals through skills-based interventions. The role blends office and community visits, with protected 40-hour weeks and shared on-call duties.
You will use recovery-focused coaching, role-play and behavioral strategies to support individuals with SPMI and co-occurring SUD/MH conditions. Travel between sites is required, and EHR documentation is essential.
Full time employment working with the Assertive Community Treatment (ACT) multidisciplinary team assisting patients to achieve recovery goals. Utilizes skills building techniques such as role-play, modeling, reinforcement, shaping, and coaching to build recovery tools for patients with co-occurring severe persistent mental illness (SPMI) and substance use disorders (SUD).
Weekly work hours and pay are not dependent on billable hours and are not dependent on grant funding. 40-hour work week with on-call, weekend/holiday hours that are shared with other team members. Requires working with patients both in the office and in the community. May provide cross-training to ACT team members as appropriate to the position.
$22.50-$27.00 DOE
Employees can receive up to $520 on the quarterly bonus.
Minimum bachelor’s degree in a human service related field. LSW preferred.
