Job summary

North Country Community Mental Health seeks an ACT Team Leader/Supervisor to provide clinical leadership for the ACT team, ensuring eligibility and program requirements are met while delivering direct services. The role focuses on reducing hospital utilization and improving psychosocial outcomes through person-centered goals.

Qualifications

• Master’s degree in Social Work or related field with licensure to supervise.
• Two+ years of supervisory experience in behavioral health.
• Qualified as a mental health professional per MDHHS/standards.
• Must possess a valid Michigan driver’s license and good driving record.

Responsibilities

• Supervise ACT program staff, activities, productivity, and resource utilization.
• Lead person-centered planning and integrated treatment plans for co-occurring disorders.
• Coordinate after-hours on-call schedule and team consultations.
• Collaborate with MI Programs (outpatient, Clubhouse, residential) and hospital staff.
• Coordinate psychiatric and medical services for ACT clients.
• Maintain positive relationships with community providers and ensure regulatory compliance.
• Oversee program operations, records, reports, and data handling.
• Manage office supplies, vehicle maintenance, and emergency funds.
