Job summary

North Country Community Mental Health in Michigan seeks an ACT Team Leader/Supervisor to provide clinical leadership to the ACT team, ensure eligibility and program requirements, and uphold MDHHS, Medicaid, CARF and NCCMH standards while delivering direct services to clients.

The role coordinates the ACT program with the MI service system, other agency programs and community partners, supervises staff, and drives outcomes such as reduced hospital utilization and improved functioning.

Qualifications

• Master’s degree in Social Work, LPC or a licensed human services discipline with licensure.
• Two or more years of experience with persons with serious mental illness and at least two years of supervisory experience.
• Satisfactory completion of training to meet regulatory bodies; lived experiences valued.
• Valid Michigan driver’s license and satisfactory driving record.

Responsibilities

• Supervises ACT program staff, activities and resources, ensuring quality and cost-effectiveness of services.
• Participates in team planning and establishes goals for person-centered plans including integrated treatment planning for clients with co-occurring disorders.
• Coordinates on-call schedule and after-hours coverage for team members.
• Coordinates services with other MI Programs and community partners; maintains positive relationships with providers.
• Maintains confidential program and client reports, records and data; ensures compliance with Medicaid and regulatory requirements.
• Oversees staff clinical documentation and visits clients in residence or community settings.
