InterCommunity, Inc. is seeking an ACT Community Support Specialist to provide skill building, engagement and care coordination as part of the Assertive Community Treatment Team. The role involves community-based work in homes and various settings with a focus on recovery and integration of services across behavioral health and medical care.

Qualifications

• Bachelor’s or associate degree in health or behavioral health or equivalent experience.
• Experience in recovery-focused settings is preferred.
• Ability to connect clients with community resources and services.

Responsabilités

• Participate on a transdisciplinary ACT team with daily meetings.
• Link clients to clinical, medical, social and rehabilitative services.
• Support client recovery through individual treatment sessions and coordination.
• Complete documentation including court letters, medical records and scheduling.
• Engage clients to support ongoing attendance and treatment engagement.
