Job summary

Qualifications

• Bachelor's degree in human services or four years of qualifying experience.
• Valid driver's license and auto coverage required.

Responsibilities

• Collaborate with clients and clinical teams to develop and revise treatment goals.
• Support clients with daily living needs, symptom management, employment, and housing.
• Provide case management, advocacy, and coordination of care across community services.
• Participate in assessments and treatment planning.
• Offer crisis intervention support, including some evening, weekend, and on-call hours.
• Assist with transportation as needed to ensure access to services.
