Job summary

Heritage Behavioral Health Center seeks a Community Support Specialist for its Assertive Community Treatment (ACT) team in Decatur, IL. You will deliver intensive, community-based mental health services to adults with serious mental illness in homes, shelters, and other community settings.

You will work with a multidisciplinary team to prevent hospitalization, support recovery, and build life skills. The role emphasizes flexible, person-centered care and engagement in real-world environments.

Qualifications

• High School Diploma or GED plus 5 years in mental health or social services.
• BA/BS in social work, psychology, education, or related field preferred.

Responsibilities

• Provide intensive, community-based ACT services to adults with serious mental illness.
• Participate in multidisciplinary team meetings and collaborative service planning.
• Engage clients in community settings with thorough, person-centered support.
• Assist with housing, benefits access, and transportation coordination.
• Document services and client progress promptly and accurately.
• Support clients in building daily living skills and recovery goals.
