Job summary

Renaissance Behavioral Health LLC in Cleveland is seeking a Behavioral Health Case Manager for our Assertive Community Treatment (ACT) team. You will work directly with clients in their homes and communities, coordinating services and providing wraparound support for individuals living with severe mental illness or substance use disorders.

Ideal candidates have a Bachelor's degree in a related field and may hold CDCA or LCDC certification.

Qualifications

• Bachelor's degree required in Social Work, Psychology, Counseling, or related human services field.
• CDCA or LCDC certification strongly preferred (Chemical Dependency Counselor Assistant).
• Experience working with individuals with severe mental illness and/or substance use disorders preferred.

Responsibilities

• Provide community-based case management across client homes, shelters, hospitals, and community settings.
• Coordinate services per Individual Service Plan (ISP) and link to housing, employment, healthcare.
• Deliver crisis intervention and support to stabilize clients.
• Monitor medications and participate in team care meetings and recordkeeping.
• Ensure compliance with laws and RBH policies; document treatment records accurately.
