This isn't traditional case management. This is high-impact, community-based work where you'll be part of an elite, multidisciplinary team providing intensive, wraparound support to individuals living with severe mental illness and/or substance use disorders. You'll work where your clients live—in their homes, in their communities, and in their moments of greatest need.

What Is ACT?

Assertive Community Treatment (ACT) is an evidence-based, team-centered approach that delivers comprehensive mental health services directly to individuals who need them most—those who have historically struggled with traditional outpatient care. As an ACT team member, you won't sit behind a desk waiting for clients to come to you. You'll go to them, building relationships, breaking down barriers, and providing the consistent, compassionate support that leads to real recovery.

  • Link, coordinate, and manage services that align with each client's Individual Service Plan (ISP)
  • Connect clients to vital community resources—housing, employment, healthcare, and more
  • Monitor service delivery to ensure clients receive the support they need, when they need it
Crisis Response & Support
  • Provide crisis intervention with confidence, compassion, and clinical skill
  • Support clients through acute challenges, helping them stabilize and regain footing
  • Collaborate with the ACT team to develop and adjust care strategies in real time
Medication & Treatment Support
  • Assist in monitoring medication compliance, supporting clients in adhering to treatment plans
  • Participate in clinical case meetings, staff development, and team consultations
  • Maintain accurate, confidential clinical and administrative records in compliance with agency standards
Compliance & Professional Excellence
  • Uphold all laws and regulations governing counseling, social work, and therapy in Ohio
  • Maintain full compliance with national and state standards, and RBH policies and procedures
  • Document with precision, ensuring timely, thorough treatment records and reports
