Job summary

Friend Health in Chicago seeks an ACT CADC Case Manager to deliver community-based services to adults with serious mental illness, providing assessment, treatment planning, therapy, and ACT model coordination.

You will conduct biopsychosocial assessments, connect clients with housing, medical, and vocational resources, document in EHRs, participate in interdisciplinary teams, and support crisis intervention to reduce hospitalizations.

Qualifications

• Bachelor's degree in a relevant field; Master’s preferred.
• Experience in community mental health or behavioral health settings is beneficial.
• CADC certification is required in Illinois; valid driver's license.

Responsibilities

• Provide 6–7 billable client encounters per day.
• Participate in 24/7 crisis on-call coverage.
• Coordinate discharge planning with state facilities and ACT team.
• Conduct biopsychosocial assessments to determine service needs.
