Job summary

TCN Behavioral Health Services seeks an ACT Case Manager in Xenia, OH to deliver intensive case management for individuals with serious mental illness and co-occurring disorders within the Assertive Community Treatment framework.

The role requires clinical supervision, timely documentation, and collaboration with the ACT team and community partners to ensure continuity of care and adherence to program standards.

Qualifications

• Education: bachelor’s degree in human/social services or equivalent.
• Experience: 3 years working with SMD population.
• Certifications: First Aid; CPR; De-escalation.
• Other: Valid Ohio driver’s license and proof of automobile liability insurance; OMHAS licensure and certification requirements.

Responsibilities

• Provide intensive case management under clinical supervision, meeting productivity and quality requirements.
• Meet or exceed standards pertaining to services provided.
• Accumulate required billable hours of clinically allocated time.
• Develop and maintain quality clinical charts and documentation; progress notes within 24 hours.
